旧博多上魚町（現博多区上呉服町）にある葛城地蔵尊の御札。毎年正月に30～40枚ほど版木で刷り、正月、5月、9月の地蔵の縁日（24日）に、参列者に配られる。１年間神棚などに飾り心願成就を願う家もある。〔第37回新収蔵品展〕
