江戸時代、全国の神社・神職を支配した京都の吉田家が、香椎宮の神職・御田範衡に宛てた神道裁許状。風折烏帽子と狩衣という装束を着用して神事に奉仕することを許可する内容。御田家は、香椎宮創建時に香椎廟司（香椎大宮司）に補された膳大伴宿祢範綱を祖とする家。〔第37回新収蔵品展〕
